I have also experienced this behaviour. A restart of Isadora solved it for me.
I have not yet found a way to reproduce it. In my case I was loading 'large' images and zooming/panning the viewport.

Are you images Stage size or larger (mega pixels ++)?

The osc settings allow a lot of flexibility but also can stress a system. Unless you know exactly what you are doing it it advisable to stick with the defaults which will work for 99.5% of cases.

These settings should be saved in the preference file. Deleting the preferences should be an easy fix.

What was the error message? I have been having an issue which sounds similar and I get the response "The media file could not be loaded because of an unexpected error.. May be corrupt" with error number = -108. If I don't close and re-open the patch,  Isadora crashes or stops allowing me to save, though sometimes it shoots up its own collage of the images (some of which weren't part of the scene or bin the media was in) like some kind of AI modern art project before it does so...

The corrupted image (a digital cut and paste type thing of Isadora's own making as opposed to the nothing that shows up when I simply lose the media) happened far more frequently when I added HSL Adjust actors.

It happens with different still image file types and the resolution of the images seems to be irrelevant. For me the issue started when I was using 2.5.2 and I was told to update my Isadora to the newest version; however, I still get the error and it stays with the patch if I move to another computer.

What happened to the app? i can't find it anywhere. After quite some time i need to use my 2 motrr devices again. but the app is nowhere to find. was trying to install it onto my iPhone se using iTunes 12.7.x.x (but even there i see no more app button :( whats wrong here?

Thanks for @artoo 's advice here i have tried to put the file on the right place and the software stop crashing again .
However the preview stage is still showing nothing. i have tried to test the live stream and it is the same ( even though i can preview the live stream in video capture setting ) 
I am wondering is there any way i can use 2.6.1 in this mac properly ( izzy 2.6.1, mac 10.9.5)   ?

i've noticed this kind of behaviour recently, using 3 buffers of 120 frames each. after a rehearsal session of loading and clearing and resetting quite a few times, I got a message that i've run out of RAM. (the message suggests I should manually allocate more RAM to Isadora, but OSX handles all of the RAM automatically these days, as far as i know)

To be able to help you we need further information:

1) You say that you've tried several codecs, but can you please give us a list or at least tell us one of the codecs that won't play?

2) Similarly, can you give tell us the typical resolution of the clips you've tried? 1920x1080? Or?

3) I assume you're running Isadora in fully enabled mode (i.e., not in demo mode). Is that correct? Are you running the USB key version? If so, is your USB key inserted into the computer?

At the moment, even though it says that setting the file name is optional, its necessary. You have to set a name for the upload to be shown. Sorry for the inconvenience.

Well I can't test on PC but on the Mac if AutoAdjust ist active you can move the order of the media on the right side without having to worry that the movie number or any media that has been set in the scenes get switched. 

To say it different if activated it is synced to the actual media if its of it is synced to the number.

And unfortunately it is not saved with the file.
